Brief Paperwork
Upon entering the office, we have some brief paperwork for you to fill out. If you would like to complete the paperwork prior to the first visit, click the links below.
History and Examination
A history and examination will be completed before any chiropractic treatment is rendered. An initial history will be completed before any acupuncture treatment is rendered. A brief history will be completed before massage therapy is rendered.
X-ray Studies
Your specific condition may require x-rays to rule out more serious conditions, this will be determined by the Chiropractor.
Home Instructions
Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice application instructions and avoidance of certain activities or positions.
Schedule Your Next Appointment
You will be given a convenient appointment time for your follow-up visit. Generally speaking for chiropractic care, patients are seen within 1-2 days and are provided with a complete review of findings. Treatment plans for your condition, financial plans, your first adjustment and therapy will also be discussed during this time. Acupuncture and massage treatment plans will be recommended by your therapist.
